How a Crown is Applied in Turkey?
Due to its economic prices, educated physicians and quality treatment opportunities at international standards, Turkey is frequently mentioned in the world about aesthetic dentistry practices. Crown veneer is one of the most preferred aesthetic dentistry applications in Turkey.
Local anesthesia is used for crown coatings. First, the patient is locally anesthetized. After the anesthesia, the related teeth are minimized. The reduction process is applied to the entire surface of the tooth as 360 degrees. After the reduction process, impressions are taken from the patient’s mouth for the preparation of crows. The moulding is sent to the laboratory. It will take several days to prepare the crowns in a laboratory environment. Therefore, temporary teeth are prepared for the use of the patient and adhered to the tooth surfaces until the prosthesis reaches the clinic. Temporary teeth are not solid because they are made of acrylic material. These teeth are used temporarily until permanent dentures arrive. The aim here is to diminish the sensitivity of the patient and to enable him to eat by providing the chewing function. A day or two later, the substructure of the crowns arrives from the laboratory. With this substructure, the patient’s gingival adaptation is controlled. This is a rehearsal phase. A fitting session, before the porcelain is covered on substructure, increases the success rate.
After the rehearsal, the substructure is sent back to the laboratory to be covered with porcelain. When it is coated, comes back to the clinic. At this stage, an aesthetic rehearsal is applied once again. If the compatibility of the prosthesis with the teeth and gums is successful, it is cemented permanently. In case of inability to adapt for any reason, permanent prostheses might be sent back to be controlled in the laboratory environment. After the prosthesis is permanently attached, the treatment is completed.
There are different material options for the substructures of crowns. The materials used in the substructure ensure a support under the porcelain coating. Metal and zirconium are the most preferred substructure materials. The metal substructure can be seen at the junction of the gingiva and the crown rarely. This may be insufficient to meet the high aesthetic expectations. In such patients, substructures produced from zirconium give better end results. E-max crowns are the type of crown that gives the most aesthetic result.
Metal Fused Porcelain Crowns
Metal substructures can be preferred for crowns. For these crowns, nickel alloy grey metal is used in substructure. Metal is covered with porcelain in order to obtain a natural tooth appearance. It is very difficult to distinguish the metal fused porcelain crown from the natural tooth. Therefore, it is often preferred in smile designs and implants. It is quite durable. These crowns are used to cover the molars, which are more exposed to chewing force. However, the metal structure has a disadvantage in light transmission. Although they are more durable, they do not give the desired result in the coating of the anterior teeth as they have less light transmittance property. Coating the front teeth with zirconium or E-max crowns, gives much more aesthetic results. In addition, for patients using metal-supported porcelain crowns, as a result of gingival recession, the metal substructure might appear as a grey line at the junction of the gingiva and the crown. Metal-supported porcelain crowns can be easily preferred if oral health is not neglected and prepared with the right technique.
Zirconium Crowns
Zirconium crowns are produced from Zirconium Dioxide, a powdered white metal oxide. Zirconium is used in the substructure of these crowns. Zirconium is extremely durable and resistant to wear like metal. In addition, it is a biocompatible material with the human body. Perfect harmony with the natural tooth is achieved by coating porcelain on the zirconium substructure. There is no possibility of allergic reactions since metal is not used in the substructure. Due to the white color of the zirconium metal, the light transmittance of these crowns is quite high. Therefore, they ensure aesthetically perfect results. They are one of the most preferred types of crowns for anterior teeth owing to their aesthetic appearance. When metal-supported crowns are used on posterior teeth, zirconium crowns can be preferred for anterior. In this way, aesthetic expectations can be met. The natural appearance metal-supported crowns cannot provide because of low light transmittance, can easily be achieved with zirconium crowns.
E-max Crowns
E-max crowns are produced from Lithium Disilicate glass. This is also described as all-ceramic system. It is the most pleasing type of crown aesthetically. There is no possibility of allergic reaction at all. Known as full-ceramic or glass-ceramic, it is impossible to distinguish e-max crowns from natural teeth. Compared to other types, the least abrasion on the tooth surface is applied in this type of crown. Abrasion is applied up to a maximum of 1.5 mm. It is a healthier method because the natural tooth is minimally intervened. E-max crowns are also very satisfactory in terms of strength and durability. They are, however, used less for posterior teeth that is chewing force is intense. They have an extremely slippery surface. Thus, the probability of bacteria and stain accumulation on the surface is very low. E-max crowns have a higher light transmittance than zirconium crowns, have perfect compatibility with natural teeth. What Is the Difference between Crowns and Laminate Veneers?
Crowns and laminate veneers are generally confused with each other. The patient’s suitability for crown or laminate veneer can be clarified after the physician’s examination.
The main difference between the crown and the laminate veneer application is that there is no abrading from the tooth for the laminate veneers or if necessary the abrasion is made minimum. Laminate veneers are applied only on the anterior surface of the tooth. Laminate veneers prepared in tooth form in the laboratory environment are attached on the tooth surfaces one by one. It looks like a fake nails.
However, for crown coating, the tooth must be reduced all around. They are largely separated from the laminates since crowns cover all the surfaces of the tooth.
